New patients
1. For your first visit:

Bring a list of medicines and supplements, including doses.

Bring information regarding any previous records that we need to track down.

Bring your insurance card if you want us to send it for billing for labs, imaging, or vaccines.
2. How to schedule

Online scheduling is coming, for now, please message through Charm mPHR or Charm Connect or call us.
3. How to communicate
-
For a rapid response, please text through Charm Connect.

For non-urgent issues, you can message through Charm mPHR, which is our portal.
-
You can always call if you prefer.
-
After hours, please message through Charm Connect for anything urgent. Messages through mPHR won't be seen until the next business day.
4. Payment options
-
Payment processing fees are excessive. Please setup payment through ACH to avoid the fees. You can still use a credit or debit card, but there is a 3.9% processing fee.

Membership payments can be setup for every 3, 6 or 12 months to receive a discount.

We are on the bottom floor of the building at 540 E Abriendo Ave, in Unit D.

Please use only the address (leave off the unit number) if using Google Maps.


Current patients

1. After visit folow-up

You can find a summary of your visit, which includes vitals, diagnoses, instructions, and referral information, on our portal (Charm mPHR app) under My Health - Visit summary

Don't hesitate to message your providers or staff through our portal or through the Charm Connect app.
2. Get lab results
-
Most labs take 1 - 7 days to result.

You can get your results immediately by created a Labcorp portal.

After we review them, your results will be sent to your portal under My Health - Labs
-
If your provider has comments, they will be listed on the top of the lab report.
3. Access your chart
-
You can also access other aspects of your chart through the portal, including medicines, vitals, appointments, messages, and documents through the My Health tab on the portal.

Please be an active participate in your healthcare by uploading results or notes from care you receive outside of our office through the documents tab on the portal. If you do so, please click the three dots at the top and forward the document to your provider or one of our staff
4. How to refill medicines - choose an option

On the portal: Choose prescriptions (under my health) and tap "refill"

Send a message through the portal
-
Send a message through the Charm Connect app.
Please be sure to indicate where you want to fill the medicine.
